SINQ Target-11 was in operation 2015 and 2016. It shut down due to an incident June 2016, during which the pressure drop cooling water loop increased significantly, meanwhile, activity of also tremendously. The evidence indicated a serious failure target. As this is first severe since came into 1997, some post-irradiation examinations (PIE) were performed analyze mechanisms.
